Miragel plombe 'state of the art' Beschikbare hydrogel plombes


De Miragel plombe

Een plombe ('scleral buckle') is een soort kussentje dat operatief op een netvliesscheur wordt bevestigd om de scheur af te dichten.

Een netvliesloslating wordt bijna altijd behandeld met een operatie.
Bij deze operatie wordt het scheurtje in het netvlies gesloten door van buitenaf het netvlies aan te drukken met een kunststof kussentje, de plombe. Deze plombe wordt onder een dun bandje geschoven, welke eerst rondom het oog is bevestigd. Het bandje en de plombe kunnen in principe levenslang zonder problemen blijven zitten.

'Miragel' staat voor MIRAgel hydrogel implant/explant.
In 1985 ontwikkelde de Amerikaanse oogarts Refojo de MAI scleral buckle, ook wel 'Refojo implant' genoemd.
De MAI plombe was vervaardigd uit de hydrogel soort MAI en was een prototype, dat later op de markt is gebracht door MIRA Inc., Waltham, Massachussets, USA. onder de (merk)naam MIRAgel.

Hydrogel is a network of polymer chains that are hydrophilic, sometimes found as a colloidal gel in which water is the dispersion medium. Hydrogels are highly absorbent (they can contain over 99.9% water) natura! or synthetic polymers. Hydrogels also possess a degree of flexibility very similar to natural tissue, due to their significant water content.

De MAI plombe werd zowel episcleraal als intrascleraal gebruikt (Tolentino et al. 1981; Satu Länsman 2009). 
Episcleraal wil zeggen dat de plombe op de sclera wordt geplaatst, dit in tegenstelling tot een intrascleraal geplaatste plombe die in de sclera zelf wordt aangebracht.

Het oogwit is de buitenste laag van het oog. We noemen dit de sclera of harde oogrok. De sclera geeft het oog stevigheid. Aan de voorkant gaat het over in een doorzichtig deel, het hoornvlies. Aan de binnenkant van de sclera ligt het vaatvlies. De sclera bestaat uit meerdere bindweefsellagen en elastisch materiaal. Het wordt omgeven door het slijmvlies van het oog, de conjunctiva.

MAI en MIRAgel hadden dezelfde chemische samenstelling. In de literatuur wordt veelal gesproken over 'MAI/MIRAgel material' (Memorie in verwijzing na cassatie § 87 / 90).
 

De Miragel plombe 'state of the art'

De Miragel plombe werd aanvankelijk als ‘state of the art’ beschouwd.

In 1981 verscheen een lovend artikel van Tolentino, Refojo en Schepens, 'A hydrophilic acrylate implant for scleral buckling: technique and clinical experience' (Retina 1981; 1(4):281-6) over de nieuwe hydrogel implant:

Abstract: 
A new hydrophilic acrylate implant developed for scleral buckling was used in 51 patients: as an accessory meridional piece under a solid silicone buckle in 25 and as a main buckling element in 26 (intrasclerally in 15 and episclerally in 11). Clinical observations six to 18 months after implantation showed the implants to be well tolerated in all cases. There was no infection, rejection, extrusion of the implant, or erosion of the eye coats. In two patients who had reoperations, smooth fibrous tissue encapsulating the implant including the scleral bed was present. Our observations indicate that this new material has all the characteristics of an ideal implant for scleral buckling.

In 1985 publiceerden Tolentino, Roldan, Nassif en Refojo hun artikel 'Hydrogel Implant for Scleral Buckling', Long-term Observations ' (Retina 5:38-41, 1985), waarin verslag werd gedaan van de positieve bevindingen bij 79 patiënten (82 ogen), bij wie de hydrogel implant was toegepast:

Abstract: 

The authors report long-term follow-up clinical observations of 82 eyes in which episcleral or intrascleral hydrogel implants ware used as buckles in the repair of rhegmatogenous retinal detachment. Three fourths of the eyes were observed for 24 months or more. The implant material provided a durable and effeetive scleral buckle, did not produce erosion or infection, and was tolerated well by all patients.

Discussion:
Although this study involved a nonconsecutive series of eyes and no controls, we think that this longterm follow-up study is important in reinforcing our short-term observations 1 and those of another authors that the hydrogel scleral buckling material is safe, effective, and well tolerated by the eye. This versatile implant material can be used in a variety of cases of rhegmatogenous retinal detachment, including complex cases in both young and old patients. It is readily adaptable for intrascleral or episcleral buckling techniques and maintains an effectively stable buckle without an encircling band.

Dit artikel droeg er toe bij dat de hydrogel implant, de MAI plombe en de commerciële opvolger de Miragel plombe als 'state of the art' beschouwd gingen worden.

Zo ook nog in 1991 Schepens ea in het artikel, 'Scleral implants: An historical perspective' (Surv Ophthalmol. 1991 May-Jun;35(6):447-53): "hydrogels seem to be the ideal scleral implants".

Abstract
Scleral implants are made of absorbable or nonabsorbable materials. One category of absorbable materials consists of donor tissue, either autogenous (fascia lata, plantaris gracilis tendon) or from cadavers (dura mater, sclera, fascia lata). A second category includes gelatin, reconstituted collagen, absorbable gut, fibrin, blood plasma, air, and sodium hyaluronate; of these, specially prepared gelatin seems to be the most useful. Nonabsorbable implant materials have proven more practical than absorbable implants. Solid silicone rubber is currently the most popular scleral implant material. It is soft, easy to handle, and well tolerated. Expandable implants, either temporary or permanent, are used in the form of a silicone balloon filled with liquid. Silicone sponge, also widely used, has the advantage of great softness. However, hydrogels seem to be the ideal scleral implants. The only one commercially available is Refojo's MAI implant. It is very soft and is not damaged by sutures, has a smooth surface, is molded in several sizes and shapes, and has small pores that microorganisms cannot penetrate. When saturated with a water-soluble antibiotic before implantation, this implant releases the antibiotic postoperatively fora langer time than any other implant material. Finally, surgical adhesives are useful when the sclera is too thin or weak to tolerate sutures. The best adhesive available seems to be isobutyl cyanoacrylate.


Beschikbare hydrogel plombes

De MAI plombe en de-Miragel plombe hadden dezelfde chemische samenstelling en waren identiek. Beiden waren vervaardigd uit de hydrogelsoort MAI.

Het prototype werd MAI genoemd en de commerciële uitvoering ervan MIRAgel, waarbij MIRA stond voor MIRA Inc., de fabrikant.

MAI/MIRAgel is de enige hydrogelsoort, die op de markt is gebracht.

Hof Den Bosch dd. 5 april 2022 overweegt in rov. 3.6 dat "niet gesteld is dat de Miragelplombe, vergeleken met andere beschikbare hydrogel plombes, een grotere kans heeft op zwellen of op andere complicaties".

In het deskundigenbericht in eerste aanleg vermeldt professor Hooymans dat voor alle drie de alternatieven (siliconen spons plombes, siliconen rubber plombes en hydrogel plombes) geldt dat er complicaties kunnen ontstaan. De deskundige omschrijft als voordeel van hydrogel ploinbes (zoals de gebruikte Miragelplombe) een kleinere kans op infectie na de operatie."De meest gebruikte hydrogel is Miragel", aldus de deskundige. Niet gesteld is dat de Miragelplombe, vergeleken met andere beschikbare hydrogel plombes, een grotere kans heeft op zwellen of op andere complicaties. Dat betekent dat de sitriatie zoals bedoeld in het PIP-implantatenarrest zich hier niet voordoet. Daaruit volgt dat het Sassen is die moet stellen en bij voldoende betwisting door Radboudumc moet bewijzen dat Radboudumc tekortgeschoten is, doordat de Miragelplombe in juli 1992 niet "state of the art" was. 

Het hof negeert hetgeen in de memorie in verwijzing na cassatie is gesteld namelijk dat alleen de MAI hydrogel plombe in de handel is gebracht en (commercieel) beschikbaar was.

Satu Länsman (2009) 123 vermeldt dat er destijds drie types hydrogel bestonden: PGMA, PHEA en MAI. Alleen MAI is gecommercialiseerd. Toepassing van de MAI hydrogel geschiedde zowel episcleraal als intrascleraal maar is gestaakt als gevolg van lange termijn complicaties, namelijk aanzienlijke zwelling.

Baino (2010) 124 vermeldt - evenals eerder Länsman – dat destijds drie types hydrogel uitgebreid zijn onderzocht: PGM, PHEA en MAI, dat onder de naam Miragel verkocht werd. MAI werd door vele chirurgen als de ideale oplossing beschouwd voor netvliesoperaties, mar na 10 jaar bleken excessieve zwelling en fragmentatie op te treden met risico op ernstige complicaties en de noodzaak van operatieve verwijdering. Om die redenen zijn MAI implants geleidelijk in onbruik geraakt.

Er bestonden destijds drie typen hydrogels, waarvan uitsluitend de hydrogel soort MAI is gecommercialiseerd en verkocht als Miragel. Dit blijkt uit de volgende artikelen.

Hydrogels are semisolid hydrophilic materials with the ability to absorb water. Three types of hydrogels have been tested, polyglyceryl methacrylate (PGMA), poly(2-hydroxyethyl acrylate) (PHEA), copoly(methylacrylate-2-hydroxyethyl acrylate) (MAI) but only MAI has been commercially available (Refojo et al. 1980, Tolentino et al. 1981, Schepens et al. 1991). An MAI implant bas been used both episclerally and intrasclerally (Tolentino et al. 1981 ). It was in clinical use for several years but was abandoned due to long-term complications (Schepens et al. 1991, Marin et al. 1992, Hwang & Lim 1997, Roldán-Pallarés et al. 1999).

A particular mention should be made to hydrogel implants, as they were considered superior to silicone implants for almost 20 years (from their introduction in the mid 1970s to their progressive abandonment in the 1990s), especially as they were softer and seemed to carry a lower risk of infection (less than 1 % vs. 2-5%) [51,52]. Specifically, three types of hydrogels, i.., poly(glyceryl methacrylate) (PGMA), poly(2-hydroxyethyl acrylate) (PHEA), and poly[methyl acrylate-co-(2-hydroxyethyl acrylate)] (MAI), have been widely investigated [53]. Major advantages ofhydrogels are softness, defined swelling under hydration and the potential to act as devices for the controlled release of hydrophilic drugs [54], which is an advantage over solid silicone implants in controlling infection. PGMA was found to suffer from lack of tensile strength when swollen, whereas PHEA exhibits a dramatic tendency to fragment after swelling [55]. MAI, which is commercially sold as Mirage!, offers better bulk features and can promote the formation of a strong surrounding capsule [53,54]. Although Mirage! has been considered by many surgeons as the ideal solution for scleral buckling, fragmentation of such implants can occur after 10-year follow-up [55-58], as well as buckle overexpansion [56,59], with an associated risk of severe longterrn complications (persistent pain, foreign body sensation) and the need ofre-operation for implant removal.

Since the early 1970s, three type of hydrogels, i.e. poly(glyceryl methacrylate) (PGMA), poly(2-hydroxyethyl acrylate) (PHEA) and poly[methyl methacrylate-co-(2-hydroxyethyl methacrylate)] (MAI), have been widely tested in scleral buckling procedures [10,85-91] and MAI has been commercially available under the name of Miragel for several years.

NB. De deskundige prof. Hooymans schrijft in haar rapport (blz. 2 1e alinea): "De meest gebruikte hydrogel is Miragel"), maar zij vermeldt niet welke hydrogels nog meer werden gebruikt behalve de Miragel. Er zijn geen andere hydrogels gebruikt dan MAI/Miragel. De ontwikkeling van de andere twee hydrogel soorten PGMA en PHEA is immers in het experimentele stadium blijven steken.
